Breathlessness May Be an Early Warning of Heart Problems

Breathlessness is a common complaint, but often the problem is not in the lungs. It can be an early warning sign of heart issues.

In OPD and ICU settings, a common pattern is observed:

Initially, there is only breathlessness, and eventually, the patient may require ICU care.

1. Heart diseases do not always start with chest pain.

Many patients begin with symptoms such as: - Breathlessness while walking - Needing to stop while climbing stairs - Reduced walking speed - Increased breathing at night - Swelling in the legs

These can be early signs of reduced pumping function (low EF), heart failure, or blockages.

2. The most common mistake is attributing symptoms to age.

People often think: - "My fitness has decreased..." - "It will improve with some rest..." - "I'll get a checkup later..."

Meanwhile, the heart is quietly struggling.

3. Some symptoms are particularly serious:

- Breathlessness at rest - Needing to sit up to breathe at night - Having to raise the pillow while sleeping - Heaviness in the chest with sweating

These may indicate heart failure or severe blockages.

4. Timely investigations can change outcomes:

- ECG - 2D Echo - CT Angiography / Coronary Angiography

These tests are not just reports; they show: - How weak the heart is - How much pumping is reduced - How severe the blockage is

Early detection is an opportunity.

5. Treatments have advanced greatly:

- Advanced medicines - Angioplasty / bypass surgery - Heart failure therapies

Many patients can live normal or near-normal lives if diagnosis and treatment start early.

6. Ignoring symptoms can lead to serious consequences:

- Need for oxygen - Fluid accumulation in the lungs - Repeated hospital admissions - Severely reduced pumping function

What could have been controlled early may progress to chronic heart failure.

7. In the ICU, two different stories are seen:

- Some patients come early, start treatment on time, and maintain quality of life. - Others ignore mild breathlessness and end up in emergency care.

The difference is often not in the disease itself but in the timing of decisions.

The heart gives early symptoms; ICU admission is usually the result of delayed decisions.

Ask yourself or your family: - Has anyone reduced walking? - Avoided stairs? - Stopped due to breathlessness?

And are you ignoring these as just "due to age"?
